In some circumstances, the title of a Feedback Form or Table or Figure can be changed by a document owner after the document has been shared. After the modification, the Save button doesn't appear (shouldn't appear). But the user is prompted to save the document after the change. The change is never actually saved (so no modification happens, in fact). But it appears to have changed.
To reproduce: Create a document. Add a Feedback Form and/or figure and/or table. Save and share. Now try to modify the title.
This does not seem to be the case when adding a Feedback Form using Variant process.
